Initial village placement

For the first village placement, why are some crammed in 20+ to a province and some are by themselves with just barbs? There shouldn't be that many players per province, it should be more equally spaced.

Barbs were once players that went inactive.

I havent been playing too long, but I've yet to see an inactive player turn barb.
